Introduction: A nursing diagnosis identifies a patient's actual or potential response to a health problem (Jones 2009). Nursing diagnoses are important because they provide the basis for selecting nursing interventions (Walton 2008). This care plan is the final half of the initial care plan that identified diagnoses and nursing goals with the goal of promoting the holistic well-being, mental health and independence of a 68-year-old Mr. Bertoli who has returned home from hospital after suffering a stroke. Particular emphasis will be placed on the interventions proposed to achieve Dr. Bertoli's health objectives and on the provision of motivations. This is important to justify the importance of the interventions and indicate how they can lead Bertoli to proceed healthily through the aging of his life. How nurses' attitudes influence their quality of care will also be explored. Objective 1: Mr. Bertoli will participate in personalized patient medication education with the goal of being able to understand the important aspects of his medications and why he needs them, this will happen within two weeks. Intervention 1a: The use of an interpreter or bilingual healthcare professional during the medication education presentation (Aboul-Enein & Ahmed 2006). Rationale 1a: It is necessary to use an interpreter or bilingual healthcare provider when presenting medication education to a patient who does not speak English to bridge the language barrier between the healthcare provider and the patient (Squires 2009 ). This is because generally when a person understands important aspects of their medications, the rate of compliance increases (McGraw & Drennan 2004).
