The initial task and key for my colleagues to embrace this change agent is to form a strategy for implementing change. The appropriate theory of change to apply to this project will be Kurt Lewin's theory of change. Initial implementation of change involves unfreezing, identifying the prevalence of chronic disease in the community, and mobilizing others to see the need for change (Shirey, 2013). The unfreezing phase involves performing a gap analysis that highlights inconsistencies between the desired and current state of affairs, using statistical analysis to compare the prevalence of chronic disease in the community with national data (Shirey, 2013 ). The ongoing process of unfreezing should involve a sense of urgency, so a solution is chosen and then the formulation of a plan for implementing the desired outcome follows (Shirey, 2013). According to Shirey (2013), successful change requires the creation of driving forces for change and/or the weakening of restraining forces. The moving or transition phase of Lewin's theory concerns the vision of change as a process, in this phase the individual reacts to the change and needs
